DR. JOLENE MILLER, PT, DPT

OWNER, PHYSICAL THERAPIST

Dr. Jolene Miller is a Certified Pelvic Floor Physical Therapist. She received extensive training from Herman and Wallace Pelvic Rehabilitation Institute. This certification allows her to treat a wide range of urinary, bowel and sexual dysfunctions.

She also specializes in; Orthopedic, Sports Injuries, Pediatrics and Neurological Injuries. Her patients age range from 4 months to 98 years of age.

She believes that with one-on-one treatment with our extensively trained and focused therapists every individual can be assisted to find the recipe for success and a life without pain and dysfunction.

When she is not working she enjoys spending time with her family and friends!

DR. NOAH MILLER, PT, DPT

OWNER, PHYSICAL THERAPIST

Dr. Noah Miller is specially certified by The American Institute of Balance. This Institute is lead by the world-renowned Dr. Richard E. Gains. They are the leaders in the most effective examination techniques to treat balance dysfunctions like BPPV.

Dr. Noah believes in staying current with the newest and most advanced physical therapy techniques proven to provide optimal outcomes. He also prefers to use dry needling as a method to treat soft tissue injuries (muscle, nerve, tendon) with dry needling as it’s a conservative way to rid spasms and trigger points without medication.

Dr. Noah has experience treating patients in various PT settings ranging from acute, sub-acute, and outpatient clinical settings. He believes that manual/hands-on PT coupled with a specialized care plan is the key element in rehabilitation.

When not in the office, Dr. Noah enjoys spending time with family and friends. He is very excited to be a part of the Garden State Physical Therapy Group.

DR. JOHN BAYLON, PT, DPT

PHYSICAL THERAPIST

John completed his Bachelor’s in Physical Therapy at Emilio Aguinaldo College, Manila, Philippines and went on to earn his Doctorate in Physical Therapy at University of Montana, Missoula. He has completed Clinical Affiliations in major hospitals and rehab facilities in Manila and California. He passed the National PT Licensure Board Examinations in both countries before settling in New Jersey where he worked in a variety of settings including in-patient/out-patient hospital, sub-acute rehabilitation, home health care, and pediatrics with the NJ Early Intervention System.

For nearly three decades clinical experience, he has particular interest and spent most of his career in orthopedic out-patient cases and completed various coursework and certifications through the years including Orthopedic Manual Therapy, McKenzie, Balance-Vestibular Rehabilitation, Comprehensive Sports Rehabilitation, IWA Strength Trainer Certification, Society for Golf Medicine and Performance, to name a few.

John’s approach to treatment is a skilled hands-on therapy that utilizes a variety of evidence-based physical therapy modalities including exercises and manual techniques.

DR. TIMOFEY RUBANYUK, PT, DPT

PHYSICAL THERAPIST

Timofey Rubanyuk DPT completed his undergraduate studies at the University of Connecticut and earned his Doctorate in Physical Therapy from Touro College in Manhattan, NY in 2016. Tim has extensive experience working with various patient populations, including geriatrics, pediatrics, post-operative care, and sports injury.

Pain, disability, injury and performance are all influenced by a variety of factors in an individual’s life. Tim works togethers with his patients to find and address the multitude of these contributing factors in a comprehensive and inclusive manner. Tim believes that best results come from forming an effective partnership with the patient, guiding them through their recovery and assisting them in becoming well-informed and highly motivated actors playing an integral role in their own rehabilitation process.

When not working with patients, Tim dedicates the majority of his time to his family and children. Tim is also an active Brazilian Jiu-Jitsu competitor as well as an Ice Hockey Official.

DR. SUSIE MATSUTANI, PT, DPT

PHYSICAL THERAPIST

Dr. Susie Matsutani is a licensed physical therapist in both New York and New Jersey. She wants to help patients get out of pain and achieve their highest level of function. Specializing in the Graston technique and kinesiotaping, Dr. Susie uses that experience to empower her patients to do more. She also has a level 1 SFMA certification and is working on certifications in dry needling and the Schroth method.

Dr. Susie graduated with her doctorate in physical therapy from Dominican College and bachelors in biology from New Jersey City University. Dr. Matsutani treats all conditions including vestibular and balance disorders. She has a special interest in sports and orthopedic rehabilitation. Dr. Susie has been trained with all ages from the pediatric population to the geriatric population.

When she’s not in the clinic, she enjoys traveling and coaching softball at Ramapo College where she has been since the fall of 2014.

DR. JUSTIN MAIRS, DC

OWNER, CHIROPRACTIC PHYSICIAN

Dr. Justin Mairs is a Board Certified Chiropractic Physician in the State of New Jersey and New York.

Dr. Justin Mairs began his path in healthcare by graduating from Kean University in 2015 with a Bachelors of Arts in Biology. Wanting to further his knowledge of healthcare and his passion to help others, he decided to pursue a career in chiropractic.

Dr. Mairs graduated from the University of Bridgeport School of Chiropractic in 2020. After working in multiple clinical settings, he has had the opportunity to work with the general public and athletes. He takes great pride in helping patients who are in need of becoming free of pain and have goals of restoring any functional limitation to return to their activities of daily living.

Some of his hobbies when he’s not in the office includes working out at the gym, outdoor activities, and cooking. He enjoys learning new information on the latest research in regards to patient care so he can continue to deliver the highest quality of care to his patients.

DR. HUGH KIM, DACM, LAC

LICENSED ACUPUNCTURIST & BOARD-CERTIFIED HERBALIST

Hugh Kim is a National Board Certified Acupuncturist (NCCAOM) licensed acupuncturist and certified Chinese herbalist. Having his grandfather as an oriental medicine doctor in Korea he grew up in the family clinic and was inspired to become an acupuncturist.

After graduating college, following his interest, he graduated with a Master’s degree from Emperor’s College of Traditional Oriental Medicine in California. He continued his studies to graduate with a Doctorate in Acupuncture and Chinese Medicine. He has been practicing for 20 years since 2003 and he specializes in pain management, digestive issues, women’s health, neuropathy, spine and joint-related conditions, and stress management utilizing acupuncture, herbs, cupping, moxibustion, and auriculotherapy. His goal is to be a practitioner who understands patient’s concerns and needs and guide people in the right direction in their journey of healing their mind and body.

BO JUNG CHOI, L.AC, DIPL. AC., LMT, CPT

LICENSED ACUPUNCTURIST

Bo Jung Choi, L.Ac, Dipl. Ac., is a licensed acupuncturist in the state of New Jersey and New York, who is nationally certified by the NCCAOM. After completing his Bachelor’s Degree in Biological Sciences at Binghamton University, Bo continued on to graduate with his Master’s Degree in Acupuncture at Tri-State College of Acupuncture in New York City. Bo practices three distinctively unique styles of acupuncture: Traditional Chinese Medicine (TCM) style, which focuses on finding and balancing the underlying root cause of an ailment, Acupuncture Physical Medicine (APM) style, which targets and relieves a specific muscle trigger point/s, and Kiiko Matsumoto Japanese Acupuncture (KM) style, which painlessly and precisely reduces stress on the body and regulates bodily functions. By utilizing these multifaceted styles of acupuncture and other holistic modalities, Bo aims to provide exceptional patient-specific treatments to effectively assist each patient reach their wellness goals. DR. DOROTHY LADYZHENSKY

OCCUPATIONAL THERAPIST, OTD

Dr. Ladyzhensky received her degree from the University of St. Augustine for Health Sciences. She has been a guest speaker at international conferences on the topic of occupational therapy in chronic pain management, and has presented research on improving the quality of life for individuals who struggle with Fibromyalgia (FM).

Dr. Ladyzhensky utilizes a collaborative approach when tailoring treatment plans for all of her patients. She believes optimal care is achieved through holistic and individualized intervention.

DR. CASEY PARKS, DC

CHIROPRACTIC PHYSICIAN

Dr. Casey Parks is a highly skilled and experienced chiropractor who is passionate about helping patients achieve their optimal health and wellness. With years of training and practice, Dr. Parks has developed a reputation for his ability to create personalized treatment plans that meet the unique needs of each patient.

Dr. Parks is dedicated to providing high-quality care to his patients, and he has a particular talent for developing innovative and out-of-the-box exercises that can help improve mobility, strength, and overall health. He is also skilled in a variety of adjusting techniques, including manual and instrument-based approaches, and is committed to ensuring that his patients feel comfortable and relaxed during their treatments.

Outside of the clinic, Dr. Parks is an avid skydiver, golfer, and fitness enthusiast. He believes that staying active and engaged in physical activity is an essential component of maintaining overall health and wellness, and he often shares his passion for these activities with his patients.

Dr. Parks received his undergraduate degree from Coastal Carolina University, where he studied anatomy and physiology, kinesiology, and other related topics. He then went on to earn his doctorate from the University of Bridgeport, where he gained a deeper understanding of chiropractic principles and techniques.

DR.SEJIN PARK, DACM, L.AC

LICENSED ACUPUNCTURIST & BOARD CERTIFIED HERBALIST

Dr. Sejin Park is a board certified, licensed acupuncturist and herbalist. Dr. Park graduated from Pacific College of Oriental Medicine and holds a Doctoral of Acupuncture and Chinese Medicine, Master of Science degree in Traditional Oriental Medicine and dual bachelor’s degree in Professional Studies and Mathematics. Dr. Park has also studied abroad in Taiwan for apprenticeship to further gain knowledge and experience of the ancient Chinese medicine. He also has travelled to Guatemala through religious medical teams to help out villages in need for medical assistance. He is currently focusing on pain management.

Dr. Park has an EMT background and is a US combat medic veteran, helping the sick and wounded. Seeing lot of soldiers go through pain, suffering and depression inspired him to become a holistic practitioner to perform various different modalities to manage pain and psycho-emotional disorders. Even till this day he performs with the same professional motto “Combat Medics never stand taller than when they kneel to treat the wounded saving lives in the midst of utter chaos”

Courtney Winter L.AC., MSAOM

ACUPUNCTURIST

Courtney is an almost 20 year veteran of the healthcare field. She started working as EMT while completing her Bachelor of Science in Biology. After finishing her bio degree, she continued her studies achieving a Master of Science in Acupuncture and Oriental Medicine from FLSAOM at New York Chiropractic College. As a part of her studies, she interned in Hang Zhou China and Canandaigua’s Veteran’s Affairs Medical Center. An acupuncturist for over 10 years, she joins us with extensive knowledge of the best acupuncture treatments for painful conditions and loves treating stress and anxiety. Courtney loves helping people achieve their fullest potential through acupuncture. She has a gentle technique honed through years of origami, knitting and spinning her own yarn.

DANIEL CARTWRIGHT, OTR/L

OCCUPATIONAL THERAPIST

Daniel is a registered and licensed Occupational Therapist in New York and New Jersey.

Daniel graduated from Pace University. He has experience working in pediatric and adult outpatient settings. He also has experience working with children, adolescents, and young adults in special education, focusing on autism and related disabilities.

Sarah Rose Lockenvitz B.S, P.T.A

Physical Therapist Assistant

Sarah Rose Lockenvitz is a licensed Physical Therapist Assistant in both New York and New Jersey. She has been around the physical therapy industry her entire life. She played soccer for 19 years, starting at the recreation level, continuing to play through college and ending her career as a semi-professional for FC Copa. Physical therapy was an integral part in her ability to continue to play her sport. She graduated from Adelphi University in 2017 with a baccalaureate degree in Exercise Science, followed by a degree from a two year P.T.A program at Kingsborough College in 2020.

Her hands on experience includes working in assisted living facilities in geriatrics with a focus in treating Parkinson’s, Alzheimer’s, Dementia, and balance disorders. Sarah Rose joined the Garden State team in early 2024, looking to continue to expand her scope with post surgical and sports intervention.

Her goal is to help each patient as a whole, while not breaking them down into “pieces” to treat their diagnosis separately from the individual. She is dedicated to helping her patients return to the sports they love and the activities they cherish in their daily lives, all while minimizing pain and enhancing their functionality. 

Alexandra Sacco OTR/L

Occupational Therapist

Alexandra is a registered and licensed occupational therapist in New Jersey. Alexandra received her Bachelor’s degree in public health at Stockton University and earned her master’s degree in occupational therapy at Bay Path University. Alexandra specializes in orthopedic and neurological conditions with a strong focus in post-operative care. Alexandra strives to use the most advanced occupational therapy techniques to improve mobility, restore function and reduce chronic pain in people of all ages. Her motto is that “a disability should not define a person’s capability” and her goal is to improve her patients’ quality of life so that they can live life to the fullest.

Charles Parker, PT, DPT

Physical Therapist

Hello! My name is Charlie and I am a Doctor of Physical Therapy. Originally born and raised in Brooklyn, NY, I relocated to New Jersey with my family in 2010. From 2014 to 2018, I attended Nova Southeastern University in Tampa, FL, where I received my doctorate degree. Prior to joining the amazing team at GSPT Group, I provided a variety of physical therapy services to patients of all ages with orthopedic, neuromuscular, and/or neurological injuries or diagnoses, primarily serving outpatient and geriatric populations. My treatment approach is both patient-centered and rooted in evidence-based practices, focusing on individualized care plans to address the unique needs and personalized goals of each patient. Sessions may often incorporate therapeutic exercises and activities, balance training, and pain management techniques. I believe in combining physical therapy with a collaborative and holistic approach to help patients reach their goals, improve quality of life, and promote overall wellness.

Outside of the clinic, I enjoy spending time with my family and friends, as well as my beautiful Siberian husky, Jade. In addition, I have been actively involved in sports my entire life, especially basketball. I consider myself fortunate to have been able to compete at the collegiate level, which eventually led to coaching high school boys basketball. Currently, I am an assistant coach at my local high school, where I have been a member of the basketball coaching staff since 2014. I am excited to be part of the GSPT Group family, and I cannot wait to meet you!

Danielle Duval, M.S., OTR/L

Occupational Therapist 

Danielle is a dedicated occupational therapist with over 10 years of experience in both pediatric and adult rehabilitation. She holds a Master’s degree in Occupational Therapy and is certified by the National Board of Certification in Occupational Therapy. Danielle’s extensive expertise is complemented by her passion for wellness and fitness, stemming from her personal journey as a fitness enthusiast and participant in USBF figure competitions. Danielle adopts a holistic approach to treatment, recognizing the interconnectedness of physical, emotional, and environmental factors in her patients’ recovery. She specializes in fine motor dysfunction and has completed advanced coursework focused on upper extremity dysfunction and pain management. This knowledge allows her to design tailored therapy programs that empower patients to achieve their goals and improve their quality of life.

Danielle believes in fostering a supportive and encouraging environment for her patients, guiding them on their journey to optimal health. With her unique blend of professional knowledge and personal experience, she is committed to helping individuals of all ages regain independence and confidence in their daily activities.
